    This listing contains mandatory, standard court forms that werenew or revised in October 2000 by the Judicial Conference Forms Committee.

    THE MANDATORY USE OF STANDARD COURT forms in circuit court became effective Jan. 1, 2000, for criminal and juvenile cases, and July 1, 2000, for civil cases. The information in the accompanying list was taken from the Wisconsin Court System online General, Criminal, Traffic, Civil, Small Claims, and Family Court Forms index, located at www.courts.state.wi.us/forms/ccformsindex.pdf. This online index now contains mandatory forms that are new or revised as of October 2000.

    A list of mandatory forms that were new or revised as of Jan. 1 or July 1, as well as information on how to use the online index to the state's standard court forms, was published in the October Wisconsin Lawyer. The Wisconsin Lawyer will print a list of additional new or revised mandatory forms as they become available.
